Staying Safe—Safety Planning

Your Mental Health Is Important...

Identify triggers:

You and your therapist or Case Manager should work together to identify situations, events, or emotions that may trigger the client’s crisis.

Warning signs:

You should identify physical, emotional, or behavioral warning signs that indicate you may be in crisis.

Coping strategies:

You should identify coping strategies that have worked for you in the past, such as mindfulness, deep breathing exercises, or physical exercise.

Support network:

Support network: You should identify individuals you can reach out to for support, such as friends, family members, a therapist, or your Case Manager.

Emergency contacts:

Emergency contacts: You should identify emergency contacts, such as a crisis hotline or a mental health professional, that you can call if you need immediate assistance. You can always reach out to the Clinical Team of EYES INC of NJ as well. Our number is 1.417.932.2732  (417 WE CARE 2).

Safe environment:

You should identify a safe environment you can go to if you feel unsafe, such as a friend’s house or a public place.


Your should identify resources that can provide you additional support, such as support groups offered at EYES INC of NJ, or online resources.

Review and update:

Your safety plan should be reviewed regularly with your Counselor or Case Manager to ensure it is up to date and effective in helping you stay safe.

It is important to note your safety plan is not a substitute for professional mental health treatment. If you are in crisis, you should seek immediate assistance from a mental health professional or emergency services, or contact EYES INC of NJ.
